Many serious accidents have happened because a machine was turned on when it should have been turned off to allow for maintenance or repair. These incidents are typically caused by energy that was not released properly. Lock-out tag-out (LOTO) systems can prevent these incidents by ensuring that the machines remain off until the person who is authorized releases them.
The LOTO system consists of a number of locks tags, locks, and other equipment that are used to secure, isolate or block the machine from electrical sources. All "affected", "authorized", and "qualified" personnel must be instructed on the use of lockout/tagout devices. These devices include, but aren't limited, to locks, chains and padlocks. Each device is identified and labeled, containing the date and time of application, the name of the person authorized, and the reason for the tag-out or lockout (repair or maintenance, for example). It should be clear that these devices can't be removed until a complete LOTO procedure has been carried out.
